Dear QESymposium Participant
We hope that you found the QESymposium informative and useful. To help us learn from your experiences, we would be most grateful if you could share your feedback with the Organizing Team by completing the survey below. This should take no more than 10 minutes of your time.
The survey will be complemented by a small number of short individual interviews, and these will explore issues raised in the survey.
This survey is being conducted by the Oswaldo Cruz Foundation (Fiocruz) Brasilia and the Norwegian Institute of Public Health. Participation is voluntary. If you choose to participate, you can withdraw your consent at any time without giving a reason. There will be no negative consequences for you if you do not want to participate or later choose to withdraw.
This survey is anonymous and you will not be identifiable in any dissemination of the survey findings.
The QESymposium organisers will analyze the data. We will securely store the survey and other data for 1 year and after this time they will be destroyed. We respect your trust and protect your privacy and therefore will never share these data with any other parties.
By completing this survey, you agree that we will process your data in line with the privacy policies of Fiocruz Brasilia and of the Norwegian Institute of Public Health.
